MINI IMPLANTS FOR RELIABLE FIXATION OF REMOVABLE DENTURE
Situations when a patient wears a removable denture on the lower or upper jaw occur quite often.
However, when using such an orthopedic design, patients experience significant inconvenience:
- the prosthesis does not fit well in the mouth
- the prosthesis moves when eating and speaking
- the structure rises from its place when chewing viscous food.
Not every patient can afford to make a modern fixed prosthesis on implants using the “ALL-ON-6” or “ALL-ON-4” technology due to their financial situation or due to medical contraindications for health reasons.
The ideal way to carry out high-quality removable prosthetics and get a stationary and securely fixed prosthesis is to install mini implants.

Kinds
Mini dental implants
are used in the following cases:
- For installation of removable structures. After implantation into the jawbone, titanium pins have good primary stabilization, which makes it possible to immediately use them after surgery.
- In orthodontics (orthodontic mini implants) to correct malocclusion. Orthodontic structures, when combined with mini-implants, guide teeth into the correct position much better. In combination with braces, for example, implants can speed up treatment.
- Widely used in preparing teeth for prosthetics. If lost teeth cannot be restored immediately, then to avoid displacement of the remaining teeth, mini implants can be installed in place of the lost ones.
MINI IMPLANTS FOR PROSTHETICS
Mini implants are significantly different from standard implants. They have many positive aspects:
- mini implants are cheaper than standard ones.
- mini implants are installed without an incision, through a puncture in the gum, which is not traumatic.
- mini implants can be installed for diabetes and periodontitis (periodontal disease)
- To reliably stabilize a removable denture, only 4 mini implants are installed.
- mini implants are reliable due to their monolithic design and last up to 25 years.
The technique of fixing a removable denture with mini implants was developed more than 30 years ago. During this time, more than 9 million of these orthopedic products have been installed in the world and vast clinical experience has been accumulated.

Installation process
The dentist first examines the mouth to determine the location and number of pins needed. They should be located at a distance of 5 to 8 mm from each other.

Then the doctor administers local anesthesia and proceeds directly to the installation:
- The is punctured and a pin is screwed into the bone.
- is fixed .
- is attached using locks that ensure stable retention.
In the upper jaw, the number of pins required can be up to six. This is due to the presence of maxillary sinuses and loose bone tissue in the immediate vicinity.
When installed on the lower jaw, the implantation of four pins is sufficient. They are installed in the anterior section, where the bone is denser. With proper care of the system, the prognosis is favorable.
In cases of pain in the postoperative period, it is recommended to take painkillers. In the first few days, experts advise eating only soft foods.
ADVANTAGES OF PROSTHETICS BASED ON MINI IMPLANTS AT THE BIONIC DENTIS CLINIC
The installation of removable dentures on mini implants and conventional removable dentures has huge differences. The differences are manifested both in the comfort of use and in the appearance of the prostheses.
COMPARATIVE CHARACTERISTICS OF CONVENTIONAL REMOVABLE DENTURES WITH DENTURES FIXED ON MINI IMPLANTS
CONVENTIONAL COMPLETE DENTURE | PROSTHESIS WITH MINI IMPLANTS |
moves when eating | does not move when eating |
moves when talking | does not move when talking |
moves when the tongue moves | does not move when the tongue moves |
food gets stuck under the denture | No food gets under the denture |
not reinforced | reinforced for strength |
fragile | durable thanks to modern materials |
massive | miniature and does not take up much space |
heavy | lightweight, thanks to modern alloy |
covers the palate | does not cover the palate |
doesn't look natural | looks like your own teeth |
may accidentally pop out of the mouth | cannot accidentally fall out of the mouth |
causes nausea | does not cause nausea, since the size of the prosthesis is not large |
rub the gums | do not rub the gums |
require a long time to get used to | getting used to the dentures occurs in 2-3 days |
These are the undeniable advantages of a removable denture over a conventional complete denture.

Care
Caring for mini-implants and crowns fixed to them is not much different from caring for regular teeth. However, there are still some features. In addition to brushing your teeth with a special brush 2 times a day, it is advisable to use an irrigator to clean the neck of the artificial tooth root from plaque.
Once every 3 months, check the tightness of the fixation of the dentures to the implants and the correct fit of the removable denture to the gum to avoid disturbances in the distribution of the load on the teeth. It is also recommended to have the denture base professionally cleaned at your dentist’s office every 3 months. This is necessary to remove plaque and food particles that can cause gum inflammation and rejection of mini-implants.

REVIEWS OF BIONIC DENTIS PATIENTS ABOUT PROSTHESIS WITH MINI IMPLANTS.
Over the 14 years of using the removable prosthetics technique on mini implants, we have listened only to positive reviews from our patients. The fact is that most often people come to us who have used conventional removable acrylic dentures for several years before. They were fully able to find out how much this is not a modern and massive structure. The second type of patients who come to us for mini-implant prosthetics are patients who have recently had a simple full denture made, but they cannot use it due to an increased gag reflex and the constant feeling of nausea that they cause.
Our patients note that the period of adaptation and getting used to a removable denture on mini implants is minimal, usually it takes 2-3 days.
Mini implants for stabilizing dentures make this design immobile, which eliminates chafing of the gums and oral mucosa. While conventional complete dentures require a long period of getting used to, which can take 6 months.

Stage of prosthetics with removable prosthesis –
The duration of prosthetics is only a few days. Immediately after the implants are installed, impressions are taken, from which a dental technician will make your prosthesis in the laboratory. In some cases, impressions can be taken before the implants are installed, thereby reducing the time required for prosthetics - in this case, you will receive a prosthesis immediately after the operation (in some cases, your old prosthesis can also be used for the first time).
The advantage of this type of prosthetics is that if your current prosthesis is made well and no later than 2-3 years ago, then you can use your old prosthesis. For this purpose, a fixation system will be additionally built into it. After installation of the prosthesis, the patient receives precise instructions on hygiene and how to properly remove and put on such a prosthesis.
Options for fixing the prosthesis on mini-implants –
Any of the two systems for fixing a removable denture listed below will consist of 2 elements. The first element is the metal head of the implant (abutment) protruding above the surface of the mucous membrane. The second element is a silicone matrix with its holder, which are built into the prosthesis in the projection of each implant.
When putting on the prosthesis, the silicone matrices tightly grasp the heads of the abutments and, thus, the prosthesis is securely held. In order to then remove such a prosthesis, you will need to apply a force equal to lifting a mass from 400 to 2000 grams (this range is associated with different options for clamps that can be installed).
- Fixation on ball-shaped abutments (Fig.4) –
This is a traditional fixation system that has been used for decades.
Ball abutments look like metal balls that protrude above the oral mucosa. As we said above, a mini-implant can immediately have a permanent spherical abutment, or the latter can be screwed into the implant after surgery. A recess/socket is made on the inner surface of the prosthesis in the projection of each mini-implant, into which a metal holder is inserted along with a silicone matrix in the form of a rubber sealing ring. When putting on the prosthesis, the silicone matrix tightly holds the head of the spherical abutment in the area of its neck.
- LOCATOR fixation system® (Fig. 5) –
This new, more patient-friendly system for fixing removable dentures on implants is the most modern to date.
It is implemented on equatorial abutments (the latter differ from spherical abutments in their shape). Mini implants for the Locator® System have a diameter of 2.5 mm, but they are also available in diameters of 4.0 mm. This fixation system is more convenient for patients in terms of the comfort of putting on and taking off the prosthesis, allows them to visit the dentist less frequently for maintenance of the prosthesis, and can also be used when the axes of the implants do not coincide with each other - up to 40 degrees. In addition, when using equator abutments there is no problem of wear and the need to replace abutments. Photos of the clinical application of this system are shown below (Fig. 12-13).
Mini implants: before and after photos
1) Clinical situation No. 1 – (with a fixation system on ball abutments)
2) Clinical situation No. 2 – (with a fixation system on ball-shaped abutments)
3) Clinical situation No. 3 – (with Locator® fixation system – on equatorial abutments)
The concept of stabilizing removable dentures on mini-implants is the so-called “soft load”. Ring attachments located in the prosthesis provide retention (retention) of the prosthesis, as well as simultaneous distribution of the chewing load between the implants and the tissues of the prosthetic bed. If the silicone matrices are removed, the load from the prosthesis will be distributed only to the soft tissue surrounding the implant, and the implants themselves will not be loaded.
In this way, the doctor can regulate the load on the implants. For example, in the lower jaw, implants can almost always be given an immediate load and, accordingly, silicone matrices will immediately be inserted into your prosthesis to hold the prosthesis (24stoma.ru). However, in the upper jaw the bone is softer, and therefore the primary stability of the implants will be worse here. Therefore, for the first time, attachments will not be installed in your new prosthesis, so that the implants will heal without loading on them.
SPECIAL IMPLANTATION TECHNIQUE IN THE BIONIC DENTIS CLINIC FOR INSTALLING MINI IMPLANTS.
In our clinic, to install MINI IPLANTS, we use modern techniques using navigational surgical templates.
Our patients undergo a 3D scan of the jaw using a computed tomograph.
Implantologists in the program process the tomography, choosing the optimal location for installing implants in the jaw bone. This takes into account the location of blood vessels and nerves. The program allows the doctor to see all the features of the anatomical structure of the patient’s jaw and not affect the anatomical structures.
Based on the planning results, a navigational implant template is made.
The template is placed on the patient's jaw during the implantation operation, and through it, the doctor accurately places the implants in a predetermined location.
This operation is completely safe and painless. Implantation is carried out without incisions, through mini punctures in the jaw bone.
Advantages of our implantation technique:
- less volume of anesthesia drug
- implantation takes less time; installation of 4 mini implants usually takes about 30 minutes
- no swelling after surgery
- there are no incisions in the gums and no healing
- no pain in the postoperative period
- the removable denture can be used immediately
Installation of mini implants using an implant navigation template can be performed on patients of any age and with diabetes mellitus who have contraindications for conventional implantation.
Our clinic has extensive experience working with elderly patients, whose condition during surgery is monitored especially carefully.

Advantages and disadvantages
Like any other system designed to restore teeth, small implants have their advantages and disadvantages.
The main advantages include:
- a small number of contraindications;
- the ability to implant into bone tissue of various quality and volume ;
- short installation period . Typically, the procedure lasts about 30 minutes;
- during surgery, trauma to soft and bone tissue is minimized. The operation is carried out by point installation in the gum and does not require detachment of the mucous membrane;
- To carry out the procedure, local anesthesia is sufficient ;
- practically no rehabilitation period , since the operation does not involve stitches;
- the ability to load the structure directly after installation;
- high survival rate;
- long service life;
- the cost of such models is slightly lower than standard implants;
- Implantation can be carried out at any age .
In addition to the advantages, systems of this type also have some disadvantages :
- due to their small size, they are rarely used to restore a single defect in teeth that are constantly involved in chewing ;
- the design feature makes it possible to carry out prosthetics mainly on the lower jaw ;
- high probability of product deformation if the jaw is injured .
Everything about sinus lifting in dentistry: complications, indications, progress of the operation.
Not everyone can get dental implants; here are some contraindications that will force you to choose a different method of prosthetics.

PROSTHETICS WITH MINI IMPLANTS ACCORDING TO THE STANDARD OF THE MINISTRY OF HEALTH OF THE RF
In Russia, an industry standard for prosthetics has been developed by the Ministry of Health of the Russian Federation. This regulatory act contains information on the requirements for:
- dental clinic and staff
- dentist's qualification level
- technique used for treatment
- materials for prosthetics
These rules regulate all aspects of the treatment process and are quite strict. Unfortunately, only 15 percent of dental clinics in Moscow and the Moscow region adhere to these rules. This is due to the fact that to reduce the cost of the prosthesis, clinics use fewer mini-implants, which are required for this type of prosthetics, or use materials that do not have a registration certificate from Roszdravnadzor of the Russian Federation. Such prostheses can cause harm to the patient's health and cause an allergic reaction.
The dental clinic "Bionic Dentis" clearly and relentlessly follows the standard of the Ministry of Health of the Russian Federation when installing a removable denture on MINI IMPLANTS when using prosthetics for its patients.
When choosing a clinic and specialist for prosthetics, carefully approach this issue. Such a prosthesis installed in a reliable clinic will serve for many years and will not create problems for its owner.

In what cases are mini-implants used, and what restrictions exist?
As stated earlier, mini implants are intended to be used in unusual situations. These include:
- Lack of significant bone tissue, which leads to the need for osteoplasty;
- With deep lesions in the area of periodontal tissue and significant loss of teeth due to inflammatory processes in the mucosal area;
- The alveolar ridge is quite thin;
- For fixation of temporary dentures in case of abnormal position of teeth;
- For fixing removable plates;
- If the position of teeth in a row is excessively high;
- To improve the bite or install braces;
- If there is a significant divergence of units or their increased crowding;
- If there are no units on the opposite side;
- In case of absolute lack of teeth, as a supporting structure for a prosthesis;
- If necessary, to recreate lost units for elderly patients.
In addition to standard fixation in the area of gingival tissue, small implants can be installed in the alveolar process or in the palate, which helps expand the possibilities of their use.
There are also restrictions on their use:
- Serious disturbances in the functioning of the cardiovascular system;
- In the presence of endocrine diseases associated with impaired insulin production;
- Insufficient blood clotting;
- In the presence of malignant formations;
- If the thyroid gland is not functioning properly;
- Psychological disorders, stressful conditions;
- Severe renal dysfunction;
- Alcohol abuse;
- Pathological dependence on drugs;
- The period of bearing a child;
- If you have sexually transmitted diseases;
- Deep disturbances of metabolic processes;
- Infection of the mucous membrane;
- Abuse of tobacco products;
- Tuberculosis;
- Diseases for which any surgical intervention is prohibited.
PROSTHETIC REINFORCEMENT WITH MINI IMPLANTS IS A DISTINCTIVE FEATURE OF PROSTHETICS AT BIONIC DENTIS
The special distribution of chewing load on removable dentures requires mandatory reinforcement of the frame to impart strength to the structure. Yes, this structural feature of the orthopedic product makes it more expensive, but without it it will not last even 3 months.
We make the frame of the prosthesis on mini implants from a highly durable and lightweight metal alloy that does not corrode and does not add weight to the prosthesis.
IN THE PHOTO: REMOVABLE DENTURE STABILIZED WITH MINI IMPLANTS. AT THE BASIS OF THE PROSTHETIC THE METAL FRAME IS CLEARLY VISIBLE FOR STRUCTURE RIGIDITY
What are mini implants?
These are small structures that allow you to recreate the functionality and shape of natural teeth. Although these products are identical to traditional dental implants, they have specific features that affect their practical use.
For example, mini-implants have a small diameter, the parameters of which do not exceed 2 mm. This makes the process of fixing them easier and speeds up the recovery period. In terms of survival rates, these implantation systems also resemble conventional implants made of titanium material.
As for the shape of such structures, they are more like screws, which ensures ease of installation. In turn, thanks to the special thread and monolithic design, the ability to be firmly held in the tissues of the jaw is achieved. Prosthetic devices can be fixed to mini-implants after full contact of the latter with bone tissue has been achieved.
The developer of the above implantation systems is Dr. Sendax from the USA. It first became known about it in 2004. After a long search for ways to restore teeth for patients with small bone tissue, Sendax introduced the world to a universal technique - mini-dental implantation.
This innovation has enabled thousands of people with jaw conditions to have their teeth reconstructed. At the same time, in terms of reliability parameters mini-implants are identical to traditional designs, but this technology is implemented in an easier way, without causing significant discomfort to the patient.

Reviews about microimplants in orthodontics
Patients respond well to the installation of microimplants.
So, for example, if you need to pull back the “sixes” and “sevens” and free up space in front, such structures are irreplaceable. To do this, microimplants are installed on the lower jaw. Before the procedure with mini-screws, anesthesia is given, which makes everything painless. After implantation, the jaw may ache a little, but painkillers are usually not needed. As a result, patients get used to the weight of braces and microimplants and no longer notice them.
Microimplants also help with distal occlusion. In order for the teeth to move back more quickly and the gap between them to be closed, microimplants are implanted into the upper jaw. The installation is painless because anesthesia is given before the procedure. According to reviews, you only feel it being screwed into the gum.
There are also negative reviews about installing implants. This is due to the fact that after several days of wear they can become loose, and then you have to take them off. This is why it is important to make sure that the doctor has relevant experience and read patient reviews.
"BIONIC DENTIS" IS A RELIABLE PREMIUM CLASS DENTAL CLINIC SINCE 2006 IN ZAMOSKVORECHYE
Our clinic has been operating since 2006 and during this period we have installed more than 1,500 removable dentures fixed on mini implants.
Our clinical experience is one of the largest in the Russian Federation and CIS countries.
The level of service in the clinic and the professionalism of the doctors allow us to confidently say: our clinic is the best choice for patients who have chosen implant prosthetics.
Flaws
Photo: Appearance of mini implants
The disadvantage of mini implants is that the small size of the structures does not allow them to fully replace real teeth.
If we compare mini implants with root implants, they are not suitable for installing full-fledged dental structures on them (dental crown, bridge) with a load on the implant.
This can be explained by the fact that if a large chewing load is given to the mini-construction, then the thin mini-implant will simply pierce the bone.

REAL STORY OF A PATIENT: INSTALLATION OF MINI IMPLANTS AND MODERN PROSTHESIS
Patient Evgenia Dmitrievna, 51 years old, came to the dental clinic with complaints of an increased gag reflex after the installation of a conventional complete denture, unsatisfactory fixation of the removable denture, discomfort and rubbing of the gums with the denture. The patient was fitted with a complete removable denture about 5 months ago in a state dental clinic, but could not get used to it, as it causes vomiting.
Diagnosis: Complete edentia
Treatment carried out:
- installation of mini implants on the upper and lower jaws
- installation of dentures fixed on mini implants on both jaws
Duration of prosthetics: 2 weeks
Duration of implantation operation using a surgical template: 40 minutes
Cost of work: 580,000 rubles
Orthopedist and implantologist: Petr Vladimirovich Ozerov
An excellent aesthetic and functional result of prosthetics was achieved. Prostheses are installed that do not cover the palate and do not cause a gag reflex. The dentures are securely fixed in the oral cavity, do not move when eating, and there is no rubbing of the gums. The patient is completely satisfied with the result of the work.
MDI
Today, MDI mini-implants are considered one of the most advanced developments in the field of dentistry. The creator of the Mini Implant (MDI) method was the American doctor Sendax, who developed a new system in 1998 together with employees of the IMTEC company. And since 2004, this implantation technique has spread throughout Europe, earning universal status.
Mini MDI implants
Externally, mini MDI implants are made in the form of miniature pins made of titanium alloy with a diameter of 1.8 mm, and also have a spherical head and a ring-shaped fastening. During the installation of the pin, an O-shaped ring located directly on the prosthesis tightly covers the head and fixes its position. Thanks to the special design of titanium screws, their installation does not cause severe trauma and the precision surgical technique is performed more smoothly. As a result, the pins implanted into the gum rise slightly above it, creating a reliable foundation for installing artificial crowns.
To make the prosthesis in the jaw more reliable, mini-implants are most often installed on 4 pins with a distance of 5-8 millimeters between them. The exact dimensions of the artificial root are determined by a specialist based on the structure and density of the patient’s bone.
In general, MDI mini denture implants can be used as either a temporary or permanent replacement for lost teeth. They are convenient, first of all, as a more effective alternative to classic removable dentures, since they completely eliminate any mobility during chewing and speech. Compared to full-size types of implants, their smaller version is characterized by greater comfort during the postoperative period, and even if the patient feels any discomfort in the first few days after implantation, mild painkillers will help eliminate it.

Mini implants: types and prices 2020
For mini dental implants - types and prices are indicated by us for 2020, based on an analysis of offers from Moscow dental clinics of economy class and mid-price category clinics.
It should be noted that the choice of types of mini-implants is very limited, because... Not all implant manufacturing companies produce them. Let us say right away that for prosthetics with removable structures on implants, we primarily recommend Osstem mini-implants (Korea). But for fixed prosthetics with bridges - Roott basal implants (Switzerland), which are similar to mini-implants, but much more reliable.
Mini-implants Osstem (Korea) –
The Korean company Osstem produces several types of mini-implants (MS series, Fig. 18), which already have permanent abutments. The first option is used with a narrow alveolar ridge for implantation in the area of the mandibular incisors. The second option is used to improve the fixation of removable dentures. The third is for temporary prosthetics.
Calculation of the price of a removable denture on 4 mini-implants –
- Surgical installation of 4 mini-implants will cost 60,000 rubles. (15,000 rubles per unit), plus about 30,000 rubles will cost a prosthesis made of ordinary acrylic plastic. Thus, the turnkey price will be about 90,000 rubles. If you want to make not an ordinary massive prosthesis made of acrylic plastic, but a slightly elastic Acry-free prosthesis, which has a lighter and thinner base, then the turnkey price will be about 105,000 rubles (including installation of implants).
Mini implants AlphaBio (Israel) –
The Israeli company Alpha-Bio is known for producing high-quality, economy-class implants, which are quite widely represented in Russia. There are 2 types of mini-implants in the line (ARRP and ARRC), which differ from each other only by the presence or absence of a permanent abutment.
- ARRP - this monolithic mini-implant already has a permanent abutment. It is used for implantation in the narrow alveolar ridge in the area of the lower incisors, as well as for prosthetics of the upper lateral incisors. It is also suitable for a one-step implantation technique with immediate prosthetics.
- ARRC is very similar to the previous one, except that it does not have a permanent abutment. This type is used to improve the fixation of partial and complete removable dentures (for this, a ball-shaped abutment is screwed into the implant).
The cost of installation and prosthetics on these implants is similar to the cost of Osstem implants (see above).
Implants Straumann (Switzerland) –
The Straumann company has implants with a small diameter of 3.3 mm, which can be used as mini-implants for fixing a removable denture. The implants from this company are among the most technologically advanced and high quality in the world, so you can be sure that they will last a long time.
The cost of removable prosthetics on Straumann implants (using the modern Locator fixation system) will range from 160,000 to 180,000 rubles.
ROOTT implants (Switzerland) –
This type of implants can be used for fixed prosthetics. Roott implants (Fig. 21) can also be classified as mini-implants, because their diameter starts from 3.5 mm. Most often, the implants of this company are called basal. This is due to the fact that, firstly, they are installed in the deeper basal layers of the bone, which are not subject to atrophy. Secondly, these implants have a bicortical type of fixation in the bone, which is much more reliable than the fixation of classical implants.
These implants also already have a permanent abutment and are intended for immediate prosthetics with a permanent bridge, which is fixed already on the 3rd day after surgery. However, they have limitations in their use - these implants are recommended to be used only in the absence of several teeth in a row (for example, 2-3 central incisors of the lower jaw).
Cost of prosthetics with crowns on Roott implants –
- Installation of 1 Roott implant together with prosthetics with a metal-plastic crown will cost 35,000 rubles.
"Full construction". For example, if three teeth are missing in a row, the cost of the entire structure will be 115,000 rubles (the price includes all expenses, including the installation of 3 implants and the manufacture of a metal-plastic bridge made of 3 crowns). Because prosthetics are done immediately after the operation - a bridge can initially be made only from metal-plastic (due to its lower weight). You can use this prosthesis for up to 3 years, but already 1 year after the operation the metal-plastic bridge can be replaced with a metal-ceramic or ceramic one.
